Silver Lining works with state-funded home service programs that help individuals remain independent and safe at home, while avoiding nursing home placement. These programs are available to any person who requests services and meets all current eligibility requirements. The programs fund assistance with household and personal care tasks.
State-Funded Programs are Designed for:
- Individuals under 60 with severe disabilities who require support with daily activities.
- Adults aged 60 and older who seek assistance to maintain their independence at home.
Program types:
Community Care Program eligibility:
- Individuals aged 60 and older seeking assistance to maintain their independence at home with cost-effective alternatives to nursing home placement.
- Program Eligibility:
- To be eligible for the state funded program the individual must meet certain eligibility requirements for the CCP, such as:
- Being 60 years or older.
- Being a U.S. citizen or eligible non-citizen.
- Being an Illinois resident.
- Having non-exempt assets of $17,500 or less.
- Needing long-term care services to prevent nursing facility placement (determined by the Determination of Need or DON assessment).
- Being Medicaid eligible.
- To be eligible for the state funded program the individual must meet certain eligibility requirements for the CCP, such as:
Department of Human Service/ Rehabilitation Program Eligibility:
- People under 60 with severe disabilities need assistance with daily activities, seeking to stay in their homes and avoid nursing facilities.
- Program Eligibility:
- Be under age 60 at the time of application unless in the AIDS or Brain Injury Medicaid Waiver Program.
- Have a significant disability lasting 12 months or longer, or for the duration of life.
- Be at imminent risk of nursing facility placement.
- Have applied, cooperated and obtained a decision on Medicaid eligibility unless already on Medicaid or spend-down
- Require services in the home costing the same or less than nursing facility costs.
- Be a State of Illinois resident with U.S. citizenship or show proof of legal entry into the United States.
- Have assets under the asset limit which is different for individuals under age 18 and those 18 and older.
- Score the required points on the Determination of Need (DON).
Family Caregiver Program:
- If you are a family member caring for your loved one you can qualify to work and get paid as a family caregiver
- Caregiver Eligibility: As the family caregiver, you'll need to meet requirements like:
- Being at least 18 years old.
- Having requisite education or experience (e.g., high school diploma or GED).
- Being authorized to work in the United States.
- Passing a background check.
